Acute Left Ventricular Ballooning: Tools to Differentiate Hypertrophic Cardiomyopathy with Outflow Obstruction from Neurohumoral Takotsubo Syndrome

چکیده
Despite considerable interest in the syndrome of acute left ventricular (LV) ballooning, its pathophysiology has remained ill-defined. In this review, we explore observational data describing two etiologies LV ballooning: neurohumoral classic Takotsubo Syndrome (TTS), and severe unrelenting outflow tract (LVOT) obstruction patients with obstructive hypertrophic cardiomyopathy (HCM). We describe clinical presentation varying these presentations, how echocardiography cardiac catheterization may help differentiate between etiologies, detail differences management. highlight significant overlap as well key differentiating features conditions, aim to improve diagnostic awareness accuracy appropriately tailor therapy.

Takotsubo cardiomyopathy and left ventricular outflow tract obstruction.
Takotsubo cardiomyopathy often presents to the cardiac catheterization laboratory masquerading as acute ST-elevation myocardial infarction (STEMI). Some of these patients present in shock secondary to dynamic left ventricular outflow tract (LVOT) obstruction. The typical patient is an elderly, hypertensive female with sigmoid deformity of the intraventricular septum. The management of hemodynam...
متن کاملSurgical management of left ventricular outflow obstruction in hypertrophic cardiomyopathy
Hypertrophic cardiomyopathy is the single most common form of inherited heart disease. Left ventricular outflow tract obstruction (LVOTO) is a recognised feature of this condition which arises when blood leaving the outflow tract is impeded by systolic anterior motion of the mitral valve. In an important minority of patients, breathlessness, chest pain and syncope may result and persist despite...
متن کاملLeft ventricular outflow obstruction induced by tamponade in hypertrophic cardiomyopathy.
Echocardiographic abnormalities of valvular movement described in patients with pericardial effusions have included systolic anterior motion (SAM) of the mitral valve. Published illustrations have shown, however, "pseudo-SAM" rather than true SAM. We report a patient with asymmetric septal hypertrophy whose echocardiogram during tamponade showed true SAM, which was no longer apparent and could ...
متن کاملApical ballooning syndrome complicated by acute severe mitral regurgitation with left ventricular outflow obstruction – Case report
BACKGROUND Apical ballooning syndrome (or Takotsubo cardiomyopathy) is a syndrome of transient left ventricular apical ballooning. Although first described in Japanese patients, it is now well reported in the Caucasian population. The syndrome mimicks an acute myocardial infarction but is characterised by the absence of obstructive coronary disease. We describe a serious and poorly understood c...
متن کاملManifestation of Latent Left Ventricular Outflow Tract Obstruction in the Acute Phase of Takotsubo Cardiomyopathy
Objective Left ventricular outflow tract (LVOT) obstruction is a complication in 15-25% of patients with Takotsubo cardiomyopathy and sometimes leads to catastrophic outcomes, such as cardiogenic shock or cardiac rupture. However, the underlying mechanisms have not been clarified. Methods and Results We experienced 22 cases of Takotsubo cardiomyopathy during 3 years, and 4 of these 22 cases wer...
